At Langdon Prairie Health, we take steps to assure that your medical records are kept confidential and secure. We require a patient’s written authorization to release medical records. You must be 18 years of age or the parent or legal guardian of a minor.
You may request copies at any time once you have provided us with written authorization. Please note that a separate authorization is required for each request.
Tips for getting your records:
- Fill out the form as completely as possible
- Include the name and address the records should be released to
- Be as specific about the information you'd like released
- Mail the authorization form to the appropriate location
